Minimal rename of @huggingface/transformers → sillytavern-transformers to publish under https://www.npmjs.com/package/sillytavern-transformers.

package.json: Updated name fields in both root monorepo and packages/transformers/
CI workflows: Updated pnpm --filter references in documentation and publish workflows
Tests: Updated MODULE_NAME in bundles.test.js
