- Removed duplicate words ("derived") from a few kernels' license

- Homogenized license comment block in kernels/zen/3/bli_gemm_small.c
with format of all other comment blocks.
Details:
- Removed four trailing spaces after "BLIS" that occurs in most files'

- Added UT copyright lines to some files. (These files previously had

- Converted most C preprocessor macros in bli_param_macro_defs.h and

- Changed obj_t-initializing macros in bli_type_defs.h to static
