SMLoginItemSetEnabled launches the helper immediately, including while the main app is already running. Skip that re-open so hides=YES cannot shove the UI into the background, and wait on the launch condition under a lock with a timeout so a missed wakeup cannot leave the helper stuck.
Also show Manage Tunnels on reopen when no window is visible.
