Summary

setupPlan always includes install_skill action even when installation is already aligned, which adds noisy/no-op planning noise.

Evidence

src/application/SetupPlan.ts:35-41 always appends { kind: "install_skill", ... } unconditionally.
In contrast, Hopper installation is conditionally included by installHopper.

Impact

Setup output claims an action that may already be complete, lowering trust in setup plan accuracy and making idempotent runs noisier.
