Closes #654.

Document the stable, complete tools/list inventory, including unavailable tools, and the absence of list-change notifications on target/provider transitions.
Direct agents to result.tool_availability in the default binary_session response for per-tool reasons, remediation, and negotiated client features. Remove obsolete capabilities/family/paging instructions from the installed skill and its native guide.
Align contributor guidance with complete discovery and session-scoped availability.
Bump the skill revision to 25, regenerate package/product metadata and the completion manifest's skill digest, and verify the installed skill matches the complete shipped source rather than freezing its previous version number.

Runtime schemas and catalog membership are unchanged.

Verification

Actual CLI doctor against isolated Cursor configs using the manual JSON from #658: exact current version exits 0/healthy; @latest, older version, and unversioned controls exit 1/unhealthy. Config bytes unchanged.
Actual pinned SDK 2.3.1 stdio MCP against the built CLI and the public Node executable: canonical 116 tools and 116 availability entries before/open/close; target-related reasons change; zero list-change notifications; unavailable tool calls retain target_unavailable with remediation. Removed arguments are absent from the advertised schema and return the unchanged default result when supplied.
npm run test:focused -- tests/boundary/filesystem/setupSkill.test.ts tests/boundary/mcp/serverIdentity.test.ts tests/boundary/mcp/mcpDoctor.test.ts tests/conformance/readmeConsistency.test.ts: 18 tests passed.
npm run docs:generate, npm run evidence:generate, npm run docs:check, npm run check:fast, focused Oxfmt and git diff --check: passed.

No real Hopper/Ghidra verification was performed: this PR corrects documentation, the shipped skill, and its generated identity. Runtime verification used isolated temporary configuration and a public executable; no private target data is included.
